LEARNING THROUGH SPORT LTD BOOKING TERMS AND CONDITIONS

1. The full cost of your booking must be paid at the time of booking. If full payment is not made within 24 hours of your booking then your booking will automatically be cancelled.

2. Spaces in each age group are limited therefore bookings are taken on a first come first serve basis.

3. You will be notified if your booking cannot be accepted for any reason.

4. All course fees must be paid in advance, are non-refundable and non-changeable. The only exception to this is if we can re sell the unwanted place and in which case a refund will be issued with a £5 admin deducted from the amount due to be refunded back.

5. Cancellation - Learning Through Sport LTD reserves the right to cancel or postpone any session for any reason. In the event of which, either a credit will be given or an alternative session will be offered by lieu.

6. Learning Through Sport LTD reserves the right to exclude any child whose behaviour we deem inappropriate. Parent/carers will be informed and notified of this decision and no refund/credits will be issued for any child/children excluded from a session or course.

7. All Learning Through Sport LTD coaches are police checked through the criminal records bureau (DBS checks).

8. On occasions, Learning Through Sport LTD or their representatives may take photographs/filming's for promotional purposes only.

9. I agree to receiving texts and/or emails regarding Learning Through Sport Limited sessions and clubs. If not please email learningthroughsportuk@gmail.com to be removed.

10. Learning Through Sport LTD do not accept responsibility or liability in respect of any loss or damage to personal property or effects.

11. The details provided on this booking form will be held on file for the Learning Through Sport LTD administration purposes only.

12. I confirm that my child will have the following

• My child will always have shin pads with them.
• My child will always have a drinks bottle clearly labelled with their name with them.
• My child will always have hand sanitiser clearly labelled with their name with them.
• My child will be wearing suitable clothing for the weather forecasted.
• My child will have sun block applied before attending and will also have a sun hat.

13. I give consent If my child has an accident, then they will be treated by a first aider and I will be informed of the situation as soon as possible. If there is a situation where my child needs urgent medical treatment and I am unavailable I give permission for a member of staff from Learning Through Sport LTD to sign any consent forms necessary for treatment on my behalf, if the delay in getting my signature is considered by the doctor to endanger my child’s health and safety.

14. I give my permission and consent for my child to take part in contact sports sessions and matches for Learning Through Sport LTD.
